Post Graduate Year 2 Rotations
During the PGY-2 Pediatric Surgery rotation, the resident will have responsibility for the surgical care of pediatric patients, including neonatal through adolescent. Under supervision, the resident will typically perform pre- and postoperative evaluations.
- Demonstrate skill in the diagnosis, evaluation, and surgery of pediatric patients including but not limited to:
- Branchiogenic anomalies
- Pectusexcavatum and carinatum
- Emergency respiratory diseases of the newborn
- Congenital diaphragmatic hernia
- Congenital esophageal anomalies
- Hypertrophic pyloric stenosis
- Atresia, stenosis and other intestinal obstruction
- Meconium ileus
- Necrotizing enterocolitis
- Hirschsprung's disease
- Meconium plug syndrome
- Duplications of the gastrointestinal tract
- Anorectal anomalies
- Liver and biliary tract disorders (e.g. biliary atresia)
- Abdominal wall hernias and defects (e.g. omphalocele)
- Tumors of childhood
- Child abuse
- Pediatric appendicitis and Mechel's diverticulitis
- Demonstrate the need for diagnostic modalities
- Preoperative management, i.e., medical evaluation, nutritional management, evaluation of biopsy results, determining the need for and obtaining consultations and, attend and present at appropriate conferences
- Postoperative management
The resident will achieve detailed knowledge of the evaluation and management of the pediatric surgical patients. The resident will learn to critically evaluate and demonstrate knowledge of how to access today’s scientific information.
- Demonstrate thorough knowledge of embryology, anatomy, and pathophysiology of the pediatric patients in relation to surgical diseases.
- Thorough understanding and knowledge of ACLS and BLS standards to a child of any age.
- Describe how to handle airways and breathing for a child of any age.
- Demonstrate in depth knowledge of indications, management, and complications of ECMO in the pediatric population.
- Discuss the principles of preoperative and postoperative management, including fluid and electrolyte management, pain control, wound care, drain management, and postoperative care in pediatric patients.
- Demonstrate understanding of potential surgical complications. Know the management of these complications.
- Develop well thought out evidence based treatment plans for simple patient care issues.
- Develop the habit of regularly reading about each surgical problem he or she encounters.
The resident will provide care that is compassionate, appropriate, and effective for the treatment of the pediatric surgical patient. The resident will develop manual dexterity and be able to develop and execute patient care plans appropriate for their level of training.
- Demonstrate proficiency at conducting history and physical exam on a child of any age.
- Demonstrate proficiency in starting an IV and drawing blood on a child of any age.
- Demonstrate knowledge and management of fluid and electrolyte problems in newborns and pediatric patients.
- perform gastrostomy tubes, pyloromyotomy, reduction of intussusception, appendectomy, and inguinal herniorrhaphy in children over 1 year of age, excision of branchial cleft sinus and cysts, thyroglossal duct cyst, and circumcision under supervision.
- Demonstrate proficiency in inserting central lines, NG tubes, and urinary catheters in pediatric patients of all ages.
- Be adept in assisting in cannulating and decannulating patients for ECHMO.
- Efficiently complete patient care activities.
- Learn to maintain close communication with senior residents and attendings.
- Maintain timely, legible, and thorough medical records.
- Provide safe patient care.
- Be reliable and honest.
Practice Based Learning
The resident will investigate and evaluate his or her own patient care practices, appraise and assimilate scientific evidence, and initiate improvement in patient care.
- Analyze, critique, and review surgical literature as it applies to practice based medicine.
- Use information technology and other multi-media resources to increase medical knowledge and assist in patient care.
- Develop a personal program of self study and professional growth with guidance from the teaching staff and faculty.
- Apply knowledge of study design and statistical methods to appraise the literature and conduct scientific research.
Assume responsibility for teaching and supervision of subordinate surgical house staff and medical students.
- Prepare for, and participate in, M&M.
Interpersonal and Communication Skills
The resident will learn to demonstrate interpersonal and communication skills that result in effective information exchange with patients, their families, and professional associates.
- Demonstrate the ability to respectfully and effectively develop a therapeutic relationship with patients and their families.
- Work well with faculty, nurses, and other support staff.
- Gather essential information from patients to be able to document patient encounters accurately and completely.
